2025-2026 Student-Athlete Physical and Eligibility

Overview:

Brevard Public Schools requires all student athletes to follow the athletic clearance process prior to participation in any athletic activities.

If you are new, you will need to create an account in Home Campus (AthleticClearance.com). This account will be used throughout your student-athlete’s middle and high school career.

Keep your username and password for future use!

Each year you will be required to log in and update your account with updated documents, certificates and signatures.

Step 1: Gather The Required Forms and Documents

You will need to download, print, complete, and re-digitize (scan or take a picture with your phone) the following forms before continuing to Step 2 where they will need to be uploaded. 

Coach’s note: This part of the process can be tedious, I know. There’s just no easy way to create a more efficient system when considering each family, student, and school’s unique circumstances.

EL2/Physical Form

NEW for 2025-2026. Must be filled out by a physician and signed by both the student AND a parent/guardian.

See Step 2 for a list of providers offering free or low-cost physicals.

Brevard Public Schools ECG/EKG Form

Required once upon entering middle school and then again upon entering high school.

Middle school scenarios: 7th graders or 8th graders who are new to Edgewood will need an ECG form uploaded/filed. Returning 8th graders with an ECG form on file do not need to file a new form.

High School scenarios: All incoming 9th graders or high school students (9th grade or higher) new to Edgewood will need an ECG form uploaded/filed. Returning 10th-12th graders with an ECG form on file do not need to file a new form.

Parents can choose to opt-out of the ECG in the form, but the form must still be uploaded noting that you’ve chosen to opt-out.

Additional Documents to Have Ready

You will also need to provide digitized copies of the following documents:

  • Athlete’s birth certificate
  • Athlete’s medical insurance card

Step 2: Get Your 2025-2026 Physical

Important: We were told that all new physicals must be on the updated FHSAA 2025 form in order to be valid for 2025-2026 (including summer activities).

This means your student-athlete will need to get a new physical before they’re able to participate, even if it’s been less than a year since their last one.

As mentioned, the EL2 physical form (available for download in Step 1) must be filled out by a physician and signed by both the student AND a parent/guardian.

Step 3: Upload Documents and Complete Online Clearance Forms

You and your athlete will need to log on to AthleticClearance.com where you will:

  • Upload your athlete’s EL2, ECG/EKG form, Off-Campus Athletics Permission Form, birth certificate, and medical insurance card. 
  • Provide additional medical and contact information.
  • Digitally sign your agreement and consent to the forms provided in the student athlete packet.
  • The student-athlete should be prompted to watch several required educational and safety video courses, and will need to upload certificates of completion for each. See Step 4 below for more details.

Step 4: Watch Required Educational and Safety Videos

Note: These courses are free.

The student-athlete should have been prompted to watch the required educational and safety video courses during Step 3 above, but in case they need to return to those videos later, here are separate instructions:

All student-athletes must complete several courses on the National Federation of High School Sports website. Below are directions to take these courses and obtain the certificates of completion for each.

Once completed, you will need to upload the certificates to your AthleticClearance.com account within the “EL3” section.

DIRECTIONS:

  1. Go to NFHSLearn.com.
  2. Click “Register” in the top right-hand corner of the page. If you already have an account, sign in with it.
  3. Fill in the information provided using your student email address: student#@share.brevardschools.org
  4. In “Courses” you can search for the following courses, or use the links below to go straight to them:
    1. Sportsmanship
    2. Hazing Prevention for Students
    3. Concussion for Students
    4. Heat Illness Prevention
    5. Sudden Cardiac Arrest
  5. “Order the Course”. The courses are free. (Do this for each course. Then proceed to “Checkout”)
  6. Click back to your “Dashboard” or “My Courses”
  7. You should see the course you ordered in progress with 0% Completed.
  8. Click the arrow pointing right and begin the course.The directions within the course will be embedded.
  9. When you have completed the course, download and save the certificate.You should have a certificate for each course upon completion.

Step 5: Upload Course Certificates

Once completed, you will need to upload the certificates from Step 4 to your AthleticClearance.com account within the “EL3” section.
